Reporting Concerns

BYS practices a policy of no-reprisal when any staff member or youth in residence reports an alleged breach of The Code of Conduct. The procedures for doing so also promote fairness in the investigating procedure and the final decisions regarding the disposition of the complaint.

Any staff member or youth may report an incident where they believe the Code of Conduct has been breached by talking to any staff member, in confidence, or writing a letter or e-mail. If the individual reports verbally, the staff member hearing the concern will document the concern in a memo format and have the person sign it. 


The documentation is forwarded to the Executive Director.

The Manager/Executive Director will: 


  • Re-interview the individual who brought forward the concern

  • Decide who else to interview to investigate the incident

  • Select one other staff member to participate in the interviews

  • Conduct interviews and write a report on the information gathered, the decision regarding the validity of the concern and recommendations for the next steps required to address the concern, and forward such findings to the Executive Director

If the concern is found to be unsubstantiated, recommendations for further action will include talking to the individual who brought the concern forward and the staff member who was named in the concern. If required, both parties will be asked to participate in a discussion regarding the misinterpretation of the incident.


If the concern is substantiated and, in the opinion of the Executive Director, has caused harm to a youth in residence or to another staff member, or to a community member, and warrants termination of the staff member, termination will take place immediately.

If the concern is substantiated and in the opinion of the Executive Director, warrants corrective action, The Executive Director will outline a plan of action(s) to be taken with a timeline indicating the meetings with the individual which will assess the success of the plan. 


This process will be completed within one month. Reports will be filed under Code of Conduct concerns and held for 5 years. In the case of substantiated reports, termination and/or disciplinary documents will be filed in the staff member’s personnel file.
